Who We Are
Hope Grande Prairie is a community-driven collaborative initiative dedicated to connecting individuals and families with the supports they need. From housing and mental health services to family programs and crisis assistance, our mission is to ensure that no one has to navigate challenges alone.
Our approach focuses on making help easier to access. Through our shared platform, HopeHub, local organizations can securely collaborate and share information — with consent — reducing duplication and ensuring that individuals and families do not need to repeat their story multiple times.
Hope Grande Prairie was developed based on the 100 Families Initiative from Arkansas, a proven model that has successfully transformed communities by fostering collaboration and using data to coordinate services effectively. Inspired by this success, we brought the framework to Grande Prairie and adapted it to meet the specific needs of our local partners and residents.
By uniting service providers under a shared approach, we are able to strengthen connections across the community and ensure that families receive the right support at the right time. Our ultimate goal is to build a coordinated, responsive, and resilient network of care that helps people thrive.

How This Works
HopeHub operates as a centralized, secure platform for identifying and addressing the needs of individuals and families. The process begins with a comprehensive assessment that evaluates 13 key areas of health and well-being, known as the pillars of health.
Based on the assessment, a personalized action plan is created. Each pillar is categorized as crisis, at risk, or thriving, providing a clear picture of where support is needed most.
A dedicated care team then uses this action plan to coordinate services, set goals, and establish tasks tailored to the participant’s needs. This may include referrals to local organizations, programs, or services that can address specific challenges.
Through ongoing monitoring and collaboration, the care team helps participants move from crisis to stability and, ultimately, to thriving. By combining structured assessment, coordinated planning, and community-based support, HopeHub ensures that families receive timely, effective, and holistic care — all in a secure, collaborative environment.
